Cortivet Shock Dexamethasone 21-phosphate is a very fast-acting, second-generation fluorinated corticosteroid that reduces the inflammatory response by inhibiting prostaglandin synthesis, stabilizing lysosomal membranes, reducing enzyme release, and inhibiting the migration of inflammatory cells (neutrophils and monocytes-macrophages), as well as decreasing the ability of basophils and mast cells to release histamine. It also reduces the release of toxic oxygen metabolites and inhibits the synthesis and secretion of chemotaxins (interleukin-1). These properties make it the drug of choice for all acute inflammatory processes, whether or not they are life-threatening to the animal. Because more than 80% of the drug circulates bound to plasma proteins, a reduction in these proteins—primarily transcortin—increases susceptibility to the effects of glucocorticoids. It has a direct positive inotropic effect on the heart; In blood vessels, it increases responsiveness to catecholamines and inhibits vascular nitric oxide synthase (NOS), leading to an increase in blood pressure. In cases of shock—whether hypovolemic, traumatic, hemorrhagic, or septic—the use of this drug is recommended in conjunction with appropriate fluid therapy and the administration of epinephrine. INDICATIONS FOR USE It is indicated for shock, stress, neonatal distress syndrome, traumatic brain injury, spinal cord compression, bronchospasm, allergic reactions, and all acute inflammations of the musculoskeletal system. ROUTES OF ADMINISTRATION AND DOSAGE Although all routes of administration are available, the route of choice is intravenous, as it allows for rapid bioavailability of the drug. The recommended dose is 0.2 to 0.8 mg per kilogram of body weight, and 2 mg per kilogram of body weight in cases of shock. In the latter case, repeat the dose at 3 and 6 hours. CONTRAINDICATIONS In cases where a circulatory disorder involving the capillary circulation is suspected, do not use this product or use it only under strict supervision, as it promotes thrombotic processes.
