Breath Up (Clembuterol) It is a selective β2-adrenergic agonist and, as such, acts as a smooth muscle relaxant with bronchodilator and tocolytic effects. Binding of the β2 agonist to its receptor leads to the activation of adenylate cyclase on the inner side of the membrane, which converts ATP to cAMP; CAMP, in turn, acts as a second messenger to activate specific protein kinases. These kinases activate other enzymes responsible for causing bronchial smooth muscle relaxation. They also inhibit the release of chemical mediators by pro-inflammatory cells and stimulate mucus secretion. Its therapeutic effect lasts for 6 to 8 hours, although it is not completely eliminated until approximately 60 hours after administration. INDICATIONS FOR USE The drug of choice for pneumonia, chronic obstructive pulmonary disease (COPD), respiratory failure accompanied by dyspnea, and asthma attacks accompanied by bronchial spasms and cough. It may also be used to relax the smooth muscles of the uterus. ROUTES OF ADMINISTRATION AND DOSAGE It is recommended to start with low doses due to the potential for toxic effects. As a bronchodilator, intravenous or intramuscular administration is recommended at a dose of 0.8 mcg/kg of body weight (2.5 ml/100 kg) every 12 hours until symptoms resolve, although a 10-day course of treatment is recommended for bronchial conditions. To induce relaxation of the uterine smooth muscle, a single dose of 0.8 to 1.4 mcg/kg of body weight (7–15 ml total) is recommended. CONTRAINDICATIONS Its use is contraindicated in pregnant mares that have already reached their expected date of foaling. WARNINGS At high doses, toxicity may occur, manifested by muscle tremors, sweating, and restlessness.
